2023-24 Required Pre-Participation Medical Clearance Procedures and Required Documentation

DEADLINE FOR COMPLETING SPORTSWARE AND SUBMITTING PRE-PARTICIPATION MEDICAL DOCUMENTATION:  AUGUST 1, 2023  

 

Below you will find all of the information you need and that is required in order to be cleared for participation for intercollegiate athletics at Curry College. 

**Please note that these forms are in addition to the medical forms required by Curry College Student Health Services. Curry College Sports Medicine does not receive paperwork you send to Health Services.

SportsWare Online also known as (SWOL) is our online medical record system we use for all injuries and where you can find all necessary documents to complete the Sports Medicine Medical Clearance Process. All student-athletes, first year and returning, are REQUIRED to complete SportsWare and the required forms as well as the medical history questions EVERY academic year. All waivers must be electronically signed on SportsWare.

Physical Exam Requirement

Physical Exam  Bylaw 17.1.5

ALL NEW student-athletes (all freshmen and transfer students) must have a complete physical exam within 6 MONTHS of the first day of their FIRST PRACTICE. For example, if the first practice is 8/15/2023, then the student-athlete must have a physical exam on or after 2/15/2023. 

This physical exam requirement is an NCAA by-law (Bylaw 17.1.5) and mandatory for any student-athlete interested in playing an intercollegiate sport.  We realize and apologize for any inconvenience as most insurance carriers only pay for one physical exam per year; however we cannot accept any physical exam dated more than 6 months and the student-athlete will NOT be allowed to participate until a new physical exam is obtained.

COVID-19 update: We cannot accept any virtual or telemedicine physical exams for first-year or transfer student-athletes.

Health Insurance Requirements 

All Student Athletes are required to have Primary Insurance in order to participate in athletics. There is a portion of SportsWare that shows you can upload pictures of the front and back of your primary insurance cards.

Please note: Student-athletes are not allowed to waive the Curry College insurance coverage if their insurance carrier is from outside the U.S. or if they have a Commonwealth Care or Health Safety Net Plan. Please click here for additional information. 

 

Sickle Cell Trait Testing NCAA Bylaw 17.6.4.1.11

The NCAA mandates that all student-athletes have knowledge of their Sickle Cell Trait status.

Student-Athletes have the following options for Documentation:

1.  Completion of the “Sickle Cell Verification” Form by the student-athlete’s primary care physician, which can be printed from Sportsware. Once your Primary Care Physician has completed the form, upload it to Sportsware. 

2. Show proof of sickle cell testing done at birth. All individuals born after 1994, were tested at birth. Records can be obtained by contacting the hospital the Student-Athlete was born at.  Upload to SportsWare a copy of the Student Athlete’s newborn Sickle Cell Testing results.

3. Obtain a Hemoglobin Solubility Test, done by your physician. The expense of testing is your own, but many insurance companies will cover the cost. Please provide proof of testing performed and results using the Sickle Cell Verification Form which can be printed from Sportsware. Once your physician has completed the form, upload it to SportsWare.

Student-Athletes have the following options for Obtaining a Test:

1.  Student-Athletes must request a Hemoglobin Solubility Test from your Primary Care Physician as part of your annual physical. This method is likely covered by your health insurance.

2. Once obtaining an order from your Physician, student-athletes can have blood drawn at a local Quest Diagnostics lab, if not provided at your Physician’s Office. The testing fee is $32.50. Athletes who are positive for Sickle Cell Trait will not be prohibited from participating in intercollegiate athletics.

NCAA Forms That Are Mandatory Only If Applicable 

NCAA Banned Substance Exception - ADD/ADHD, Asthma, and Hormone Therapy

Banned substances include those used to treat ADHD/ADD, Asthma and hormone therapy used in gender transition. If the student-athlete has been prescribed medication that is banned by the NCAA you must have the prescribing medical practitioner complete the Banned Substance Exception Form which can be found and downloaded from SportsWare. Once complete, please upload a copy of the documentation to SportsWare under the attachments tab.

All students have access to fill out these forms on SportsWare under attachments to download, print and have their physician fill out if applicable.

Concussion Requirements

At-home ImPact Concussion Testing (instructions in link below): On June 1, 2023, you will receive an email from ImPact Applications, Inc (check Junk Mailbox).  In the email you will have a unique code which will allow you to take ImPact at-home. This code is only good for 90 days and will expire.

At-home Concussion Education Course (instructions in link below): Please visit SportsSafety International to take the concussion education course. You must upload a certificate of completion to SportsWare once completed.

COVID-19 Updates:

Effective June 1, 2023, Curry College will no longer require the COVID-19 vaccine or booster. Public health experts advise that being fully vaccinated and boosted against COVID-19 is critical to enhancing immunity and significantly lowers the risk of getting very sick. Accordingly, Curry College is strongly encouraging all students, faculty, and staff to be up to date with the COVID-19 vaccine and booster(s). This adaptation to Curry’s COVID-19 policy aligns with the state’s acknowledgement ending the COVID-19 public health emergency in Massachusetts on May 11, 2023.

Curry College will require all enrolled students on both our Milton and Plymouth campuses to upload all vaccines to Pyramed, please email Healthservices@curry.edu with any questions.

REQUIRED ECG for Anyone who had COVID-19: 

If you tested positive for COVID-19 at any point and had cardiac symptoms, please make sure you have an electrocardiogram (ECG) after recovery done at your physician's office or cardiologist prior to your arrival at Curry College. Your PCP or consulting cardiologist should complete the ECG Verification form which can be found HERE and on SportsWare. You will not be permitted to participate without an ECG or the Verification Form completed.

** Please note the athletic training staff is out of the office until August 1, 2023. We will begin to update status' on SportsWare to "Cleared" once we return and have had a chance to review all of the SportsWare accounts.  If you play a Fall sport and do not see "Cleared" checked off by August 1, 2023, please contact us at the email below to determine what is missing. Winter and Spring sports should contact us after August 11, 2023 if "Cleared" is not checked off. We first review fall sports followed by spring and then winter.
